Normal people definitely run releases, and normal NetBSD developers want
to know exactly which one you are running before submitting bug reports.

If you do run updates, you have to be specific about which ones you have
running or they can't help you.

Every NetBSD system run sanely, is going to be a base release plus noted
specific updates.  If you stray from this, the developers are likely to
ignore any bug request you make, which makes sense.

IMHO, they should periodically build a minor update release for faster
and easier server upgrades.  However, that takes a lot of build
resources for an OS that has that many platforms.
