I use a different file for each patch, /etc/install/patch_PATCHID_REV. For  
example, file /etc/install/patch_100523_26 contains the following line:

patch_100523_26 installed on Tue Feb 29 18:24:07 MET 2000

This line is displayed by /usr/etc/showrev -p .
