[rescue] Which is bad?

no scsi expert here, but I would suspect the drive. I have seen errors that
smelled a lot like this caused by a bad or incompatible scsi drive.

The weirdness here though it that it looks like '/ c0t3d0s0' was created
correctly and its on the same drive. Maybe the platter is junko?
