I had a filling done without Novocaine when I was about twelve.
The dentist said the cavity was small so there wouldn't be any
need to use Novocaine.  However, as he drilled he found the
cavity went deeper than he thought.  As the drilling proceeded
I started feeling some pain, so I squealed a couple of times.
He stopped, told me to stop squealing, and that he was almost
finished.  He then started up again, and hit the nerve.  That
had to be some of the most intense pain I've ever felt.  I
screamed, and everyone on the floor of the medical building
came over to see if there was an emergency.

He apologized, and the filling was free.
