As a starting point, it's possible that you have a memory problem or 
something, try setting 'diag-switch?' to true and 'diag-level' (I 
think) to 'max'  it the PROM, and see if it finds any problem.

Barring that, try reseating everything (procs, ram, SCSI cables, etc) 
and see if that helps.
