For me, it would work to have three regular buttons, and either a wheel or
a nub[1] just where the tip of my thumb rests against the surface of the
mouse.  The later version 1.1 Microsoft Intellimouse is shaped
perfectly[2] for this, but you'd need to narrow the buttons in order to
get a properly-sized middle button in there.

However, I haven't had any problems getting both the wheel and
wheel-button on my Intellimouse to work under both Windows and other
5-button-mouse-savvy operating systems.  The wheel-button has even worked
under IRIX, so far.

[1] I don't know what the proper term is.  It's a bit of rubber mounted on
    a momentary-contact rocker-switch, and the entire assembly behaves
    like a wheel, except that you just hold it in one direction to scroll,
    rather than turning a wheel--sort of like a trackpoint with one axis.
[2] It, however, if not properly-shaped for left-handed usage.  I'm right-
    handed, but occasionally need to mouse left-handed on a cramped desk.
    I don't care what the marketriods at Microsoft say--the mouse is
    asymmetrically-shaped, and only fits the curve of the right hand
    properly.
