What fiber cards are you using?  In our environment (LOTS of EMC fiber storage),
the NUMBER ONE MOST IMPORTANT THING when it comes to problems like this is 
UPDATE THE FIRMWARE ON THE HBAs and USE THE LATEST DRIVERS.
