I'd suggest checking out the Kinesis keyboards.  I have one of the 
countoured keyboards, and i can't reccomend it highly enough.  Yes, it's 
expensive, but not as expensive as surgury.  Besides, this keyboard can 
potentially last you many years.  It's also mac-compatible (mine has a 
switch) and has optional pedals.  I also had a maxim keyboard.  The 
configurability and size is nice, but i really didn't like the key 
action.  Mine is now on what might as well be permanent loan to a 
friend.  On both of those keyboards, i thought i'd really miss having 
the 10 key on the board, but now that i've not had one for a while, i 
can deal without it, or get one of the seperate 10 key's.  having the 
mouse that much closer is well worth it.
