At work, I use the Cyclades console servers
(http://www.cyclades.com/products/2/ts_series) which seem to work quite
well. I can't compare them with alternative modern console servers, but
they are considerably better than the old terminal server products.

For example, give a port a name and you can ssh directly into it with
'ssh -l username:portname cyclades'. You can configure accounts that
have restrictions on which ports they can use, etc.

It's based on Linux, so is pretty flexible ... you can configure ports
in a variety of ways, configure an NFS-server to keep console output,
configure a firewall, etc.

Downside is that they're pretty expensive for what they do.
