could the sync on green also be the problem with an SGI Indy using a similar
adapter and an IBM flat pannel?

Monitor probably doesent do SyncOnGreen.  Also, if its a CG6, the
monitor sense bit isnt getting passed through, so the system doesent know
what resolution to output.

Try setting the resolution "hard" in openboot?
