MCM Electronics has these for $8.09US.  There are two shapes:
72-6771 & 72-6772.  The first is a strip with the active patch
at one end, the second is a rectangle with the active area in the
center.  Check out http://www.mcmelectronics.com.
...and these used to be giveaways 20 years ago...:(
