Sounds like a Vitec RasterFlex card. Several variants of this card
around, explained in a very good support page:

If the card is a later -32 or -HR version then you can get drivers
(including SunOS 5.x) from this page (not sure about others).
Rasterflex cards appear on ebay quite regularly and usually go for
about $50. One up at the moment without any bids (one card went last
week with 26 bids for $54):

I've got a RasterFlex-HR card which will do up to 1280x1024 24-bit,
but I've not got around to installing the drivers yet so I can't
say how quick (or not) it is....
