Thats a PGX8, almost for sure.  I dont think any of the PGX24s or
PGX32s had 13W3, and most PGX8s had HD15 as well.

2 chips = 2meg of onboard RAM.  PGX24 had 4.

I'm not sure if it will work in a PC.  Dont think so.
