Symptoms:
	PHSS_19576:
	1) HPMC or HANG
	2) NO_HW reported for disks on a PCI fibre channel adapter
	when a GSC fibre channel adapter is installed.

Defect Description:
	PHSS_19576:
	(JAGab69670)
	J5000, C3000, or B1000 will eventually HPMC when more than 2
	fibre channel cards are in the system.  The PIM dump shows
	that the card was accessing and invalid page.  The problem
	was due to incorrect handling of dma mapping failure.

	When both the A3740A (PCI) A3591A (GSC) are installed
	together on a J2240, neither of them works properly.
	The following things may occur:
	disks under the PCI card show up as NO_HW
	disks under the GSC card don't show up at all
	If any disks do show up, the machine will eventually hang
	The problem was due to incorrect handling of dma mapping
	functions.

	1. Back up your system before installing a patch.

	2. Login as root.

	3. Copy the patch to the /tmp directory.

	4. Move to the /tmp directory and unshar the patch:

		cd /tmp
		sh PHSS_19576

	5a. For a standalone system, run swinstall to install the
	    patch:

		swinstall -x autoreboot=true -x match_target=true \
			-s /tmp/PHSS_19576.depot

	By default swinstall will archive the original software in
	/var/adm/sw/patch/PHSS_19576.  If you do not wish to retain a
	copy of the original software, you can create an empty file
	named /var/adm/sw/patch/PATCH_NOSAVE.

	WARNING: If this file exists when a patch is installed, the
	         patch cannot be deinstalled.  Please be careful
		 when using this feature.

	It is recommended that you move the PHSS_19576.text file to
	/var/adm/sw/patch for future reference.

	To put this patch on a magnetic tape and install from the
	tape drive, use the command:
