Patch Name: PHSS_14406 Patch Description: s700 10.20 Cumulative Online Diagnostics Tools Patch Creation Date: 98/05/07 Post Date: 98/05/19 Warning: 99/04/14 - This Non-Critical Warning has been issued by HP. - PHSS_14406 will not work correctly when applied to any Support Tool Manager (STM) diagnostic system release with an STM version of B.10.20.09, or greater. If PHSS_14406 is applied to STM version B.10.20.09, or greater, the diagnostic system will have been regressed in lost features or functionality. It may also cause other programs such as Predictive to not work correctly. - PHSS_14406 does work correctly when applied to STM versions lower than B.10.20.09. - To determine the version of STM on the system, follow these steps: 1. Type 'swlist -l product Sup-Tool-Mgr-700'. 2. Read the STM version in the center of the line. - If the installed version of STM is lower than B.10.20.09, no further action is required. If the installed version of STM is B.10.20.09 or greater AND PHSS_14406 is installed, please follow these steps to correct the Support Tool Manager diagnostic system: 1. Reinstall the Support Tool Manger software from your HP-UX Diagnostic/Independent Product Release (IPR) media using the command: swinstall -s \ -x autoreboot=true -x reinstall=true Sup-Tool-Mgr-700 Please note that STM versions B.10.20.09 and greater will be on the HP-UX Diagnostic/Independent Product Release media dated June 1998 or later. 2. Execute the cleanup(1M) utility to remove references to PHSS_14406, since it has been overwritten by the reinstallation of the Support Tool Manager software, using the command: cleanup -i If the cleanup utility is not present, it can be obtained through patch PHCO_12140. When the cleanup utility asks if you wish to remove overwritten patches from the IPD, reply "yes". Please review the man page for the cleanup utility for additional details. - A new patch, PHSS_17887, has been released to avoid this problem in the future. PHSS_17887 provides the same functionality as PHSS_14406, but includes a checkinstall script that prevents the patch from loading on any STM version B.10.20.09 or greater. - In addition to the actions above, HP also recommends that PHSS_14406 be removed from all software depots and that it be replaced by PHSS_17887. This will reduce the possibility of additional systems being exposed to these problems in the future. When the memory log is displayed via LOGTOOL, right after MEMLOGP starts, the PDT error address is incorrect and other pertinent info will be mostly unknown. b. In cases where PDT table has been reset by PDC, and there are no entries added to the table, MEMLOGP's memory log will always be cleared every time MELOGP is restarted. This problem is in systems with page deallocation support: Emerald and Hawks. 22. SCSI Dar does not support Multiple LUN access. 23. Add recognition of EISA FDDI driver. No tools supported. 24. This problem has not been observed. 25. When a command file run from xstm encounters an error, xstm dies with the message Error: XtCreatePopupShell requires non-NULL parent 26. When mstm is run on a terminal window which has more than 80 columns available, mstm will randomly exit with a 203 exit status. 27. Added recognition/support for new hardware: Toshiba CD-ROM XM-5701TA 28. This change provides support for 6 new additional sense codes (ASC) returned by the HPC5447A disk array. sense data, is currently incorrect. This patch will correct this condition. 29. SCSICD's ldev command functional for NIO driver attached devices. 30. Enhanced error reporting in HARRAY in the event of Read Compare failure during Section 10 processing. 31. Execution of the Access Log command in SCSIDSK2 against the Quantum XP series of drives produces no error counts. 32. Extra character is displayed at the end of the firmware revision string in the "Describe" command output in scsi_dds expert tool. 33. Add C1557A DDS-3 autoloader support in both scsi_lib expert and fw update tools. 34. CPU exerciser terminated with incomplete status after about 20 minutes of execution. The termination message indicated that the program was unable to allocate memory using sbrk() system call. 36. Added feature to STM NIO_FW info tool to display EEPROM code revisions, card parity check settings, and card address. PHSS_7929: 1. SCSI_MUX fw_update tool: a. Download will fail after first block is written. b. Pass-through driver locking, unlocking and resetting has been removed. Date code feature has been added. 2. Download firmware to SCSI disk with firmware files larger than 256 KB will result in a download failure. 3. Added STM diagnostic support for NIO and EISA PSI cards. 4. In SCSI_MUX diagnose tool, all the error logs available are decoded and meaningful sentences are logged correspondingly in the failure log. 5. User would see Error Log data of SCSI_MUX info tool, if any, in Failure log and Warning Log in the activity log. 6. If no SCSI cards are in a SCSI MUX, the MUX does not show up in the map. 7. The SCSI disk expert tool will fail when an identify is done on approximately 40 blocks or more of a disk. Messages in the Activity Log will indicate that an error occurred during the open of a tlidsector file in /var/tmp. 8. All graphics devices will have a product qualifier of Graphics-Device. 9. Added support for the new SPU: 9000/7xx/V132L, 9000/7xx/V160L 10. The CPU info module will panic certain systems. PHSS_7759: 1. When (70% of) the system available free memory drops to less than 1 megabytes, the memory exerciser will terminate with incomplete status. 2. Diagnostic programs terminates with the following error message on systems where root and dump are LVM, but swap is not: *** DIAGNOSTIC MONITOR COULD NOT GRANT ACCESS TO A DEVICE DUE TO FAILURE RETURNED FROM THE DIAGNOSTIC LIBRARY ROUTINE obtain_dev_type(DSSERR 667) *** A SYSTEM DEPENDENT INTERNAL ERROR WAS ENCOUNTERED. (DSSERR 5002) *** AN ERROR WAS RETURNED BY THE DIAGNOSTIC MONITOR IN RESPONSE TO A REQUEST TO OBTAIN ACCESS TO PDEV (0/12). (DSSERR 660) *** THE SPECIFIED DEVICE COULD NOT BE ALLOCATED FOR TESTING (DSSERR 2146) The "help pdev" command with sysdiag also fails with the following error message. *** A SYSTEM DEPENDENT INTERNAL ERROR WAS ENCOUNTERED. (DSSERR 5002) *** UNABLE TO DETERMINE THE IDENTITY OF PDEV (0/12). FURTHER HELP INFORMATION CANNOT BE OBTAINED. (DUIERR 372) 3. Enhancement in Support Tools diagnose scsi_dlt module. 4. a. The error rate computed in the Maximum Coverage test of the diag_dds diagnose tool does not reflect the correct rate for threshold determination. b. Running the diag_dds diagnose tool with more than one iteration and no user queries option fails with "Aborted Command" failure message. c. Enhancement to error decoding algorithm. 5. a. User is prompted each time upon tool start-up to enter the file name for the trace logs output. This make starting up the scsi_dds expert tool a cumbersome process. b. The "Channel Hang" flag in the EEPROM log can be either a bit flag or a byte flag depending on the EEPROM log format on the device. Currently, scsi_dds displays the "Channel Hang" flag as a byte format for both. 6. The "Help" command in the cstm interface for scsi_dds, scsi_dlt and scsi_lib expert tools do not work. 7. This patch contains the STM diagnostic support of the J2804A HP ATM adapter card. This card is delivered with the software product J2805AA. 8. When user ran the disk verifier on a SCSI (TEAC) floppy drive, the tool would return incomplete status and report that the disk was not supported by the disk verifier. 9. The graphics tools will hang when they are run on a system which is booted under CDE when no-one is logged into the console 10. In SCSIDSK2: a. Added support for Seagate ST32171N and IBM DORS- 32160 disks. b. Format unit times out when executed on large capacity drives. 11. Format Unit in SCSIDSK2 times out when executed on large capacity drives. 12. This is an enhancement in disk exerciser and disk verify tool to change message that shows user where to find more information in the case where he keeps getting "insert scratch disk" prompt. 13. Added support for HPC1537A DDS in SCSIDDS diagnostic. 14. Added identification support for: a. 4X MO disks, FY96 GSY 2,4,9GB disks, Lear DDS, Timberline and Redwood Tape Libraries, DLT 4000, Edison Disk Array, Andronicus DDS. 15. Spelling error in message -- use "Selete" instead of "Select" in copyutil module. 16. Xstm will crash if a command file (which is run from xstm) attempts to write a file which already exists and the 'replace file' dialog is not disabled in the general options menu. Xstm will also crash while running a startup script if the support tool license of a system it is connecting to is going to expire within the next 20 days. Additionally, if device selection (ex: sel dev 3) is done in a command file that is executed from xstm, the selection will not generate an error but will not succeed in selecting the device. Command files executed from xstm via the run command file command, the -f option, and in a .stmrc file can all cause these problems. 17. The LAN Exerciser does not receive responses from the hosts specified in file /var/stm/config/tools/exercise/hosts.cfg. 18. a. The graphics devices will intermittently show up as UNKNOWN in the STM map when the system is booted CDE. b. When using STM for tool development, starting a tool under STM in debug does not work when the new DDE (Version 4.0) is installed. c. Hosts.cfg path name in Release Notes is wrong. d. Added enhanced information tool for LAN/Console Interface. e. Added SCSI MO icon 19. When a user runs the information tools' tape modules and then wants to look at the structure information log, they will get the addressses to the malloced space, not real device data. 20. This bug occurs in MSTM only. When an MSTM window appears that contains only scrolling text (text widget), and the user hits the "up arrow" or "down arrow" key to view the text, STM will crash. 21. When run without a terminal attached to any of standard input, standard output or standard error, STM fails to connect to the localhost. The startup command file fails, and displays the message "Aborting all open command files." 22. When running the graphics exerciser on a VISUALIZE-48 or VISUALIZE-48XP, the user may see the following error very infrequently: Neptune data signature incorrect during the multiboard test. expected = 0xaef77b1a, actual = 0xaef77b19 23. Adding Fibrechannel SCSI Mux ID Module. Defect Description: PHSS_14406: 1. Initialization of the parameters by the callers of PDC dar was inconsistent. If the right number of parameters were not initialized and passed to the PDC dar, a system panic could occur. 2. Incorrect mapping of error code to catalog message results in a negative message catalog number. This causes the UI to fail and kills the main tool window. The failure occurs when running selftests under the DDS expert tool and device encounters a selftest failure. 3. The memory decode module was not supplying the correct information. The user would see the problem everytime the Information tool or Logtool is used to view memory errors. 4. Enhancements for the memory dar and memory were added for Emerald/Jade SPUs. This included Dar enhancement to improve memory controller slot identification when master add identifyable. The Memory Information module enhanced to provide block ID information for configuration and error entries. PHSS_13774: 1. The code to check for memory error failed to increment the slot number while checking for errors. PHSS_12284: 1. New expert tool added. 2. Support for new products. 3. New information module added. 4. Added support for new firmware update tool. 5. Not enough memory allocated in data array definition causing data overrun. 6. The page deallocation routine in the Memory Access Routine library was passing the wrong parameter to PDC. 7. New MSTM features. 8. Enhancements to the Logtool. 9. Added new Scsi tape qualifier entry. PHSS_8589: 1. Additional functionality. 2. The program wasn't designed to handle firmware files larger than 256KB. 3. Kernal changes were needed to allow tlidsector to work on 10.20. These changes were not made to 10.01 and there currently are no plans to patch the 10.01's kernel to support tlidsector. 4. The "not implemented" status condition was introduce late. 5. Running DDS Information tool will return incorrect compression data in Information Log. 6. Additional functionality. 7. Additional functionality. 8. Remove un-needed log page. 9. No support for scsi_cd expert tool. 10. When parsing the Xscreens file, the graphics exerciser did not look for device files that were part of a single logical screen. 11. The products were not supported product at the time of release. 12. To get around PDC F/W problem. 13. This is a new feature added. 14. Earlier only driver "fcpmux" was scanned by STM (which happens to be entry related to the SCSI card in the MUX) but now, if no scsi cards are present, "fcpdev" is used to talk to the MUX. 15. Additional functionality. 16. Additional functionality. 17. Additional functionality. 18. Additional functionality. 19. With HP-UX 10.01 the disk array is not recognized and there are no tools for it in the 10.10 or 10.01 release. There has been some format changes for the 10.20 release in 10.20 20. Need to use "#ifndef/#endif _TLSCSIDEV_INCLUDED" clause. 21. a. PDT error address is being divided by 8 in access_mem(). b. The RESET CLEAR FLAG option in PDC_MEM must be called whenever the PDT table gets cleared. MEMLOGP only does this when there are entries in the PDT. 22. SCSI Dar modified to support Multiple LUN access for disks. 23. Additional functionality. 24. A core dump could possibly occur due to this problem, although none has been observed, due to referencing an invalid structure in memory. 25. The problem can occur when the user explicitly runs a command file with an error in it, or when there is an error in the startup script .stmrc or in the startup configuration file config.stm. 26. Coding error. 27. Toshiba CD-ROM XM-5701TA is new, previously unsupported, hardware. 28. The problem is caused by a defect in the code which returns an incorrect address in the SCSI sense data. If an error is generated by an AutoRAID device, the address returned with the SCSI sense data will be in error. This patch will correct this condition. 29. ldev command previously non-fundtional for NIO driver attached devices. 30. Read command failure during Section 10 Read Compare processing was not being properly reported. 31. These drives are DEC drives which utilitizes DEC specific algorithms for most operations, but since Quantum took over the division, have changed some functions to conform with ANSI SCSI specifications. 32. Firmware revision string was not terminated correctly. 33. Additional functionality. 34. The cause was the absent of a close() system call after the corresponding open() call. Therefore, it ran out of file descriptor resource after 10 to 20 minutes of continuous execution. 35. The cause was the incorrect interpretation of ulimit (UL_GETMAXBRK) value. As a result the program was attempting to allocate more than the per process data limit. When that happened, the system returned ENOMEM on sbrk() system call. This problem can be found on system with large amount of memory(e.g. more than 100M). 36. Only IODC data was displayed in STM NIO_FW info tool. PHSS_7929: 1. SCSI_MUX fw_update tool: a. Coding error: offset was always set to 0. b. New feature. 2. SCSI_DISK fw_update tool and SCSIDSK2 were not design to handle files larger than 256 KB. 3. No previous PSI diagnostic exist in STM. 4. Added new features in the tool. 5. Added new features in the tool. 6. The MUX shows up once in the map for each SCSI card. If no SCSI cards are in the MUX, it won't show up. 7. Temporary files were not being managed correctly so the per-process limit of open file descriptors was reached while the SCSI disk expert tool was running. 8. Graphics devices attached to system will be identified with a product qualifier in the current device status as Graphics-Device, not the appropriate qualifier, like Graffitti, Hyper, etc. 9. The products were not supported product at the time of release. 10. Problem was caused by PDC F/W. PHSS_7759: 1. If the system is heavily running other applications, it is not unusual that the avaible memory may drop below 1M. This is not a problem. The change in the memory exerciser is to allow the test to continue even if the available free memory is less than 1M. In other words, the change is to lower the minimum memory requirement from 1M to 16K. If the available free memory drops below to 16K, the program will terminate with incomplete status. 2. The diagnostic library routine obtain_dev_type was not taking into account the output from lvlnboot. The code was expecting "Swap: ". On systems with the output similar to #lvlnboot -v lvlnboot: Volume group not activated. Cannot display volume group "/dev/db01". Boot Definitions for Volume Group /dev/vg00: Physical Volumes belonging in Root Volume Group: /dev/dsk/c1t5d0 (0/52.5.0) /dev/dsk/c1t6d0 (0/52.6.0) -- Boot Disk Root: lvol1 on: /dev/dsk/c1t6d0 No Swap Logical Volume configured Dump: lvol2 on: /dev/dsk/c1t6d0, 0 Dump: lvol3 on: /dev/dsk/c1t6d0, 1 the "Swap:" is not present. 3. Enhancement to the error decoding algorithm in the module. 4. a. Formula used in calculating the error rate is incorrect. b. When the "no user queries allowed" option is set and the diagnose tool is set to run more than one iteration, the tool will attempt to run tests which requires media not be present even when media is loaded causing an "Aborted command" error. c. Enhancement to the error decoding algorithm in the module. 5. a. Enhancement to trace log dump operation. Initial dump of trace logs will always write to default temporary file. User will not be prompted for the output file name with the "initial" dump of the trace logs upon start-up. User will still be prompted for the output file name when the trace logs dump is executed on demand by the user. b. Fix "Channel Hang" EEPROM log message display format to correctly display either a bit or byte setting depending on the EEPROM log format on the device. 6. Entering "Help" in the SCSI tape and library expert tools in cstm will give error message. The SCSI tape and library expert tools used "GeneralHelp" instead of "Help" as the help command. Tools fixed to use "Help" as the help command. 7. The products were not supported product at the time of release. 8. The cause of the problem was that the driver name "sflop" was not included in the verifier's checklist. 9. The problem here is that the tools and the mapping routines are using an ioctl GCDESCRIBE which is hanging when the system is booted CDE and no-one is logged on the console. 10. Devices were not supported product at time 10.20 released. The command uses a fixed timeout value which is too small for the new large capacity drives. 11. The command uses a fixed timeout value which is too small for the new large capacity drives. 12. When the tool attempts to access a bad hard disk drive, the driver often returns ENXIO. With just ENXIO (and not knowing what the device, e.g. MO, floppy, etc) it is dealing with, the tool could not determine if the drive is bad or because of the absence of media, so it keeps asking user to insert scratch disk. The fix is to enhance the message so that user can go elsewhere to get more information. 13. HPC1537A was not a support product at the time of release. 14. These changes are all to support new hardware. 15. Spelling error in copyutil module. 16. The problem will only occur when a command file is invoked while in xstm (via the run command file command in the file menu, the -f command line option, or when included in the .stmrc file.) Xstm will crash when a command file connects to a system whose password will expire within 20 days. Since the default scrip /usr/sbin/stm/ui/config/.stmrc connects to the local host, the problem may look like xstm begins to start-up and then crashes. This should only be seen in the last 20 days of January and July, as this is when the passwords will be expiring. 17. This problem occurs if the broadcast address on the host being tested is one which broadcasts only to hosts on a subnet, as opposed to an entire network. Example: The problem occurs if the broadcast address is 15.43.215.255 The problem does not occur for broadcast address is 15.255.255.255 18. a. CDE locks the graphics device when the login screen is up and thus STM cannot identify the graphics device. b. DDE changed it's command line parameters and how it executes. c. Added enhanced information tool for LAN/Console Interface. d. Added SCSI MO icon 19. The declaration of char pointers for the info_data data structure caused the problem. In this case we should use fixed size char array. 20. Design error in MSTM. 21. The function used to obtain the user name for the connection login relies on a terminal being connected to standard input, standard output, or standard error. When it doesn't find one, it uses an empty string for the login name, which is not valid. The login fails, and the startup script (which normally does an automatic login) terminates. 22. The test code did not correctly handle a situation where certain state is left in the graphics hardware. 23. The product was not available at the time 10.20 was released. Patch Conflicts: None Patch Dependencies: None Hardware Dependencies: None Other Dependencies: None Supersedes: PHSS_7759 PHSS_7929 PHSS_8589 PHSS_12284 PHSS_13774 Equivalent Patches: None Patch Package Size: 37280 KBytes Installation Instructions: Please review all instructions and the Hewlett-Packard SupportLine User Guide or your Hewlett-Packard support terms and conditions for precautions, scope of license, restrictions, and, limitation of liability and warranties, before installing this patch. ------------------------------------------------------------ 1. Back up your system before installing a patch. 2. Login as root. 3. Copy the patch to the /tmp directory. 4. Move to the /tmp directory and unshar the patch: cd /tmp sh PHSS_14406 5a. For a standalone system, run swinstall to install the patch: swinstall -x autoreboot=true -x match_target=true \ -s /tmp/PHSS_14406.depot 5b. For a homogeneous NFS Diskless cluster run swcluster on the server to install the patch on the server and the clients: swcluster -i -b This will invoke swcluster in the interactive mode and force all clients to be shut down. WARNING: All cluster clients must be shut down prior to the patch installation. Installing the patch while the clients are booted is unsupported and can lead to serious problems. The swcluster command will invoke an swinstall session in which you must specify: alternate root path - default is /export/shared_root/OS_700 source depot path - /tmp/PHSS_14406.depot To complete the installation, select the patch by choosing "Actions -> Match What Target Has" and then "Actions -> Install" from the Menubar. 5c. For a heterogeneous NFS Diskless cluster: - run swinstall on the server as in step 5a to install the patch on the cluster server. - run swcluster on the server as in step 5b to install the patch on the cluster clients. By default swinstall will archive the original software in /var/adm/sw/patch/PHSS_14406. If you do not wish to retain a copy of the original software, you can create an empty file named /var/adm/sw/patch/PATCH_NOSAVE. Warning: If this file exists when a patch is installed, the patch cannot be deinstalled. Please be careful when using this feature. It is recommended that you move the PHSS_14406.text file to /var/adm/sw/patch for future reference. To put this patch on a magnetic tape and install from the tape drive, use the command: dd if=/tmp/PHSS_14406.depot of=/dev/rmt/0m bs=2k Special Installation Instructions: This patch eliminates the 'sysdiag' diagnostics system and replaces it with 'STM' diagnostics system. NOTE #1: If the system that you are updating contains 'Predictive' software then you must also load the Predictive software from DART 35 (IPR9710). Failure to load the new Predictive software will cause the older Predictive software to not work at all. NOTE #2: This patch will permanently remove 'sysdiag' from the system. If you deinstall this patch, 'sysdiag' must be reinstalled manually.
