You want the FDDI/S DAS 5.0 card.  It's an OEM Network Peripherals board,
the Sun part numbers are 370-2340, 370-3143 or 605-1572.  Single wide,
dual attach.  It has a pair of SC duplex connectors on it, though, not
normal FDDI connectors.
