Likely they overthought the process, then failed to demonstrate line employees
knew the process and followed it. It is up to the certification candidate to
determine the appropriate process and defend it against challenges by the
auditors.

When I was being prepared for my ISO9001 certification interview, it was
stressed to me that I didn't need to know the answer to every question I was
being asked, I *absolutely* needed to know where the answer was located (SOP
website) and to never, EVER guess at an answer to a question.

This was at Bellcore before it was sucked into SAIC (large federal/defense
contractor), and at a small software/service organization. As I recall,
Bellcore achieved certification, the smaller company 'shifted focus' mid-way
through the process and was acquired.
