This is a common failure mode of that particular drive. I haven't
pulled one apart and fixed it, but my instinct is that the problem is
this:
Some internal plastic bit wears out, such that the drive never sees
the tray as fully inserted. The drive has a timer. When that timer
expires without the tray all the way in, the drive assumes the tray is
caught on some sort of foriegn object, and throws it all the way back
out so that the operator can correct the problem.
Those Toshiba drives tend to be loud and unreliable by that stage,
anyway, so it's best to just replace it.
