A followup:  

I realized that I had not tightened them enough.

The rule of thumb is to use the appropriate size hex wrench, and hold
the short part while tightening; tighten until your fingertips start
to hurt from the cheap pot metal biting into them.

I tightened them up and put it in, then it worked.
