I really like Covad service.  I use their SDSL for my smaller customers
that offer webmail and/or VPN access.  I haven't had any problems and
they even dropped their prices recently (mid contract even!).

For home, I'm lucky enough to have a great local outfit nearby
(Sacramento, CA area).  They're called Omsoft (http://www.omsoft.com). 
The owner even answers the phone sometimes, but regardless... somebody
answers the phone and helps you with no hold music needed!  They even
did a 'live cut' when I moved last time.  I just called them, unplugged
my servers and router, drove them to the new place (15 mins), and it was
working.  Try that with PacBell!!!  Of course they can't keep that up
forever, but it's cool for now.
