I was looking at www.mousesystems.com a few days ago.  They still seem
to
sell type 5 optical mice there.  They also had a couple of different sun
compatable optical mice.  A little expensive though.  I can email the 
mousepad.ps file to anyone that wants it.  It prints a type 3 mouse pad.
It looks easy enough to modify for type 4/5, if you know postscript.
