I can assure you that wouldn't happen. If you can't produce a social
insurance card and don't show up in the system, aside from maybe going
down to the local quick treatment clinic (where you'll pay cash) or
picked up in cardiac arrest or bleeding out by the paramedics (e.g. it's
a life-threatening emergency), you'll not be treated.
You cannot pay for extra treatment out of your own pocket in Canada to
get "moved up the list" or to see a specialist, other than perhaps for
elective procedures like cosmetic surgery or lasik or similar.
In the US if you show up with cash in hand, you'll be treated. This is
why Canadians come to the US.
