Advise them to walk, not run, from this.  I've done some work in 
healthcare and I realize the liability for failing applications can be 
tremendous.  Access and more specifically the Jet database back end 
were never designed for anything more than prototyping or small desktop 
database apps.  Things get REALLY bad when you have multiple concurrent 
hits to the database, and corruption is almost a given.  I ran into 
this with Intel LANDesk which uses the Jet back end (but can be 
upgraded to SQL Server).

So aside from the performance issues, and screwy things where the 
entire database is dumped onto the client desktop, there are very real 
issues with the database being corrupted terribly by multiple 
concurrent hits or by scaling too big (I've seen both).
