if you're close enough to the CO they can run copper.  if you are far enough
from the CO that either repeating or fibre is needed, they will most likely
repeat or run fibre.  they won't be happy about it, and it may be harder to
get them to do it for ISDN these days, but they will definitely do it for a
T1.
